Mental health needs rising in school

JEFFERSON-Depending on how you see things, the Ashe County Board of Education received a mix bag of news from the district’s Assessment, Support and Counseling Center at it’s May 1 meeting. The good news? The ASC Center will provide free mental health services to more Ashe County High students this year than ever before. The not-so-great news? The need for those same adolescent mental health services continues to grow, according to the center’s personnel.

That’s the state of affairs according to Whitney Van Sant, the district’s School Mental Health Coordinator. “I’m proud of the work we’ve done this year,” Van Sant told board members. “We’ve served more kids than we’ve ever served at any of our sites, including Watauga or Alleghany.”

The idea for the program is simple: offer on-site mental health services to high school students where they need it most. By identifying at-risk youth, the program works to reduce serious depression and mental illness and gives treatment options where they’re most likely to be used.

The program began in 2006, after Appalachian State University’s Dr. Kurt Michael and then Watauga High School Principal Angela Quick met to find a way for licensed mental health providers to serve WHS students.

The pair felt they could advance the high school’s teaching mission by addressing behavioral and mental health issues that were hindering the school’s mission by teaming with professional school staff, including counselors and social workers, already in the building.

The basic model of service, starting late in the 2006/07 school year, included consultation/education with faculty and administration regarding mental health issues, crisis intervention, risk/substance abuse assessments, and referral.

Four years ago the program expanded to Ashe County High School.

In the 2016-2017 school year, the Van Sant told the board the program has served some 178 students, or roughly a fifth of the student body at ACHS and roughly a percent of the student body at Ashe County Middle School.

  • 49 students were served in individual therapy/counseling.
  • 17 students were served in social and emotional skills groups (eight at ACHS and nine at ACMS).
  • 53 students (five percent of student body; 54 percent female; 46 percent males) served via crisis intervention with 70 documented events .
  • ASC Clinicians have served approximately 60 additional students with consultation and drop in support.

Roughly 90 percent of the year’s crisis events involved suicidal ideation, according to the ASC Center’s statistics but the majority of those were termed as “green” events – crisis events where students had low or no intent to harm themselves or others – but a small minority, seven percent, involved homicidal ideation and just under three percent of incidents involved both suicidal and homicidal ideation.

Van Sant said anytime a student expresses those thoughts, a risk assessment is performed and an individual treatment plan is charted for that student.

“Most students, even if they have great parents they can talk to, even if they have a trusted adult they’re most likely to go to a peer,” Van Sant said, telling board members most cases are referred to the center by other students.

Over the past four years the center at the high school has documented some 217 total crisis intervention events, none followed by a death or suicide.

According to a Youth Outcome Questionnaire, 66.5 percent of students “recovered” or clinically significantly improved, while 26.6 percent remained unchanged and 5.3 percent deteriorated.

Most students received Cognitive-Behavioral Therapy, and students received an average of 10-1 sessions with an average session duration of 35 minutes.

Over the past four years, the most commonly elevated clinical scales at the beginning of treatment included depression, anxiety, social stress, somatization (examples include headaches, stomachaches), attention problems, family problems and sense of inadequacy.

But because the business of treating mental health concerns is sometimes an intensive process, the ASC Center also tracks which students actually have the means, roughly two-thirds, to carry out their plans.

“Access to means is important because getting into why someone is having suicidal ideation or intent is complicated – but how they want to do it is something we can immediately address,” Van Sant said.

That’s why the program has distributed 19 lock boxes during the course of the program to restrict a student’s access to means they could then use to harm themselves.

“Sometimes they may be for locking up drugs but in all of these cases they are for locking up prescriptions in the home for students with suicidal ideation,” Van Sant said. “It’s a very good measure to take, including locking up Ibuprofen.”

And Van Sant and company said they hope to continue expanding services at Ashe County Middle School in the future, but said ASC staff must be wary of spreading themselves too thin to provide the kinds of intensive intervention for students when they need it.

“One of the things that make us effective is that we always have somebody on site,” Van Sant told board members. “Today we had three incidents in one block. That’s the first time that’s happened with crisis intervention so it speaks to the importance of having somebody on site.”


NC AFSP Chapter funds CAMS training  

The Counseling and Psychological Services Center and the Assessment, Support, and Counseling (ASC) Center at Appalachian State University are pleased to announce that the Program and Education Committee from the North Carolina Chapter of the American Foundation for Suicide Prevention (AFSP) funded a joint request to provide training in the Collaborative Assessment and Management of Suicidality (CAMS) Model. The CAMS approach, an evidence-based suicide prevention program, will be scaled up and implemented by campus and community clinicians from the aforementioned agencies along with other community clinicians who have a vested interest in preventing death by suicide. There is a clear need to provide expeditious assessment and effective intervention for those at risk for suicide, especially in rural western North Carolina where suicide rates are well above the State and National averages (CDC, 2013).

Dr. Denise Lovin (Counseling Center) and Dr. Kurt Michael (ASC, Psychology Department) are leading the effort to scale up CAMS in order to more effectively address this public health problem in the High Country. As a community of practitioners, Drs. Michael and Lovin believe that the training will better equip our clinicians to promote the safety of our college students at Appalachian and the adolescents and families who are part of three different K-12 university-school partnerships in Western North Carolina (Alleghany, Ashe, Watauga).  Moreover, CAMS and the functions of both groups align well with the mission and vision of the American Foundation for Suicide Prevention (

The generous award in the amount of $19,150 will cover the costs of bringing CAMS founder, Dr. David Jobes, to campus for in-person training along with online pre-training and support materials costs. ASC Center on pace to double number of students served

Ashe County High School’s Assessment Support and Counseling Center is on pace to serve double the number of students it did in 2012/13, according to data released by the program on Jan. 13.

“In terms of service delivery hours, we’re actually seeing almost as many individual cases up through December as we saw all of last year,” ASC Founder Dr. Kurt Michael told the Ashe County Board of Education Monday night.

In 2013, the center treated 47 individual cases, according to Michael.

“So far this year we’ve seen 41,” Michael said. “In terms of crisis service, we saw a total of 20 students and so far this year’s we’ve seen 22 individual cases. In terms of what we’re delivering if these numbers hold true, we’ll be doubling what we did last year, assuming the same patterns continue through the rest of the academic year.”



Graduate students compete in 3-Minute Research competition

BOONE—Graduate student Alex Kirk can tell you about his research on adolescent suicide in three minutes flat. He did just that at the Fall 3-Minute Research competition on Dec. 3 at Appalachian State University sponsored by the Cratis D. Williams Graduate School. And he got the judges’ attention.

Kirk, a second-year clinical psychology graduate student, won with a unanimous vote from the judges. His three-minute presentation focused on strategies for reducing the incidence of teen suicide in rural school districts. Kirk is a graduate student researcher in the Graduate Research Associates Mentoring (GRAM) program. His mentor is Kurt Michael.



ACHS mental health treatment program shows progress

Seventy percent of Ashe County High School students treated by the school’s new Assessment, Support and Counseling Center have “recovered,” or improved, according to data released by the program.

Dr. Kurt Michael, ACHS Assessment, Support, and Counseling Center coordinator told the Ashe County Board of Education on June 6 that treatment outcomes have improved at least 7 percent from mid-year data given in January.

“When I came and spoke in January, we found about 63 percent of those kids at an elevated level either were “recovered,” at a very low range of symptoms or improved,” Michael said. “Our numbers at the end of the year suggest that almost 70 percent showed that same outcome. So, seven out of 10 are showing the improvements we are looking for.”
